User Obligation

User shall have sole responsibility for determining the medical necessity of tests for which reports will be generated through Quantum Patient Assessment, Inc.'s website. Quantum Patient Assessment, Inc. shall have no obligation under this contract to provide any advice to User with respect to determinations of medical necessity, and Quantum Patient Assessment, Inc. shall not offer any such advice. Where User has determined a test to be medically necessary, Quantum Patient Assessment, Inc. can make available to User, for administrative purposes, a form letter that describes the particular diagnostic test and recites general information about medical necessity. Quantum Patient Assessment, Inc. makes such form letters available to its Users as an additional business service, included with the website usage. User has sole discretion to use such form letters or not as User sees fit.

4. Arbitration

QPA will never sell any Patient Information or information about our users. Users of the web site shall have access only to the Patient Information pertaining to their own patients. Any unauthorized or illegal use of Patient Information is strictly prohibited, and shall result in the immediate termination of your access to this web site.
